How many times have you wanted to know the name of a doctor or a medical health care provider who treats Parkinson patients? Wouldn't it be nice if you could go to a resource list that had this type of information? Wouldn't it be nice if the list included names that have been recommended by the Parkinson's community? The doctors and medical providers listed below have been recommended "by and for" our Parkinson's community, their families, friends, and/or caregivers. Listings below do not constitute endorsement by the Columbia Parkinson's Support Group. We are providing this information as "information only". Do you have a medical care service provider you would like to recommend?
You can help create this resource list of doctors and medical care providers, by nominating those medical care service provider or doctor who specifically treats Parkinson patients in the states of South Carolina, Georgia, and North Carolina. As always, we respect your privacy! If you so choose, your name will absolutely NOT be associated or mentioned in any way with the doctors or medical care providers listed. You are not limited to nominate just one medical care provider. You can nominate multiple medical care providers even in the same category. You can also add a new category. After you submit a medical care provider, we will contact their office directly to verify that they indeed want to be included on our list - again to reassure you, when we contact the medical care provider we will not mention your name.

Note: Self nominations are accepted. Verification will be done. Please nominate "only" for South Carolina, Georgia, and North Carolina.

For each provider nominated, please include the following information.

+ Practice or Type of provider - for example: Neurologist, Family Practitioner, Assisted Living
+ Mailing address / location
+ Telephone number
+ Email (if available)
+ Web site (if available)
